ONLINE ACCOUNT ACCESS – To insure the safety of your private health information and to create a web/internet account whereby you have full access to your prescription records we require that you:
1. Sign a “HIPAA ONLINE ACCOUNT ACCESS AUTHORIZATION” form at our store so we can verify your personal identification.
2. Create an account – steps:
- After selecting your Store location and clicking on the “Choose Store” button you will be redirected to a webpage to verify the store selected. If correct then click on the GREEN “Refill Prescription” button.
- Select “Create Account” link at the top of the web page and follow the online directions. See store for more information!
